How to Apply for FUAZ Post UTME Form

All applicants are advised to read the registration instructions before proceeding on registration.
On the portal home page, click on Pre-Admission Screening Registration link.

1. Type in your JAMB Registration Number and SURNAME correctly in the boxes provided and click on fetch data. The portal will retrieve your UTME details for confirmation before you can proceed to make payment.
2. Click on Make Payment to redirect you to the REMITA WEBPAY.
3. Make Payment
   • Select your ATM card type (e.g. Verve or Master Card).
   • Enter your Card Number, Expiry Date, CVV2 (the 3 digits on the reverse side of the card) and other information required.
   • You will receive a one-time token, enter it on the space provided and proceed to authenticate your payment.
4. On successful payment, you will be redirected to the login page. Incase your account is debited and the portal is unable to recognize your payment, don’t panic, go to the “Pre-Admission Screening Registration” page and click on “Click here to revalidate payment” to re-query the payment status.
5. To login, click on “Applicant Login” to enter your JAMB registration number and surname. After Login, you will have access to the Registration Form through the dashboard menu and ensure it is carefully completed online.
6. PASSPORT SPECIFICATION
   • Acceptable format is JPEG (*.jpg extension).
   • Resolution must be at least 200 DPI.
   • Dimension should not be less than 600 × 600 pixels.
   • Each photograph should capture the head and top of the shoulders so that the face takes up to 70-80% of the photograph.
   • Show the eyes looking directly at the camera.
   • Show skin tone naturally.
   • Have a uniform red background.
   • Maximum of 25KB.
7. Click Print Tab to print out the following:
   • Post-UTME Screening Registration form;
   • Payment Receipt.
